According to Nuclear industry sources, including a chief design engineer at the Fukushima plant, the meltdown at Unit 1 was already underway before the Tsunami hit. It has been revealed that at least one reactor had a catastrophic loss of coolant and partial meltdown almost immediately after the quake. This means that TEPCO lied about the plant problems resulting only from a chain of disasters, and tried to hide the ongoing nuclear crisis from regulators and the public. It also shows that the events that can lead to a severe accident at any nuke plant are potentially far more common. A simple broken pipe is believed to have started the unit 1 accident with fuel starting to melt within two hours of the quake.
